Understanding OCD
More Than Checking the Lock Twice

OCD is one of the most misunderstood conditions in mental health. It's not about being neat or liking things a certain way. It's unwanted, intrusive thoughts that feel impossible to dismiss — and behaviors or mental rituals performed to reduce the anxiety those thoughts cause, even temporarily. The relief never lasts. The cycle continues.

For many people with OCD, the hardest part is that the obsessions feel shameful — thoughts that seem to contradict everything you know about yourself. That shame keeps people from seeking help for years. It shouldn't. OCD is a recognized medical condition with effective treatments, and the content of obsessions does not reflect who you are.

If the obsession-compulsion cycle is consuming significant time in your day, causing significant distress, or interfering with your functioning — effective treatment exists.

Recognizing the Signs
Common Signs of OCD

If several of these sound familiar, a psychiatric evaluation may help clarify what's going on.

✓ Unwanted, intrusive thoughts, images, or urges that are difficult to dismiss
✓ Repetitive behaviors performed to reduce anxiety from obsessions — checking, counting, arranging, cleaning
✓ Mental rituals — praying, reviewing, mentally neutralizing thoughts
✓ Reassurance-seeking from others to quiet obsessive doubt
✓ Significant time spent on obsessions and compulsions — often 1+ hours per day
✓ Distress when compulsions are prevented or interrupted
✓ Knowing the obsessions are irrational but being unable to dismiss them
✓ Avoidance of situations that trigger obsessions
✓ Obsessions with themes of harm, contamination, symmetry, religion, or sexuality
✓ Functioning impaired at work, home, or in relationships due to OCD symptoms
Treatment
Medication and Therapy — Both Matter

The most effective treatment for OCD combines medication with specialized therapy — specifically Exposure and Response Prevention (ERP), which is the gold-standard psychological treatment for OCD.

Medication alone is rarely sufficient for moderate to severe OCD, and therapy alone may not be enough either. The combination produces the best outcomes for most patients.

Medication Management
SSRIs at adequate doses are the first-line medication treatment for OCD — often at higher doses than used for depression or anxiety. We select medications based on your history, prior treatment response, and tolerability. Adequate trials take time — OCD often requires higher doses and longer trials than other conditions.
ERP Therapy Coordination
Exposure and Response Prevention (ERP) is the most effective psychological treatment for OCD. We coordinate with therapists who specialize in ERP when therapy is part of your treatment plan. If you don't have a therapist, we can help identify appropriate resources.
Common Questions
What Patients Ask
Exposure and Response Prevention is a behavioral therapy that involves gradually confronting feared situations or thoughts (exposure) while refraining from compulsions (response prevention). It works by breaking the obsession-compulsion cycle. ERP is the most evidence-based psychological treatment for OCD and is recommended alongside medication for most patients.
Medication reduces OCD symptom severity for most patients but rarely eliminates it entirely. Most patients do best with a combination of adequate medication and ERP therapy. The goal is meaningful reduction in symptoms and improved quality of life.
OCD often requires higher doses of SSRIs than other conditions. If prior trials were at lower doses or for insufficient duration, a different trial at adequate dosing may still be helpful. If true treatment resistance exists, other medication options are available.
OCD and anxiety share features but are distinct conditions. OCD is specifically characterized by the obsession-compulsion cycle — intrusive thoughts driving repetitive behaviors or mental acts. A thorough evaluation clarifies the diagnosis.
